Dr.Kaul returned to the US in 2001 and commenced a career in the emerging field of minimally invasive spine surgery, being the first physician to successfully perform a percutaneous outpatient lumbar interbody fusion, a procedure which revolutionised the manner in which spine surgery would forever be performed. From 2002 to 2012 Dr.Kaul underwent exrensive training both within the US and internationally and was actively involved in teaching physicians the techniques necessary for the performance of the procedure he had pioneered in 2005.

In 2008 after an unplanned trip to the Democratic Republic of Congo he established the Spine Africa Project, a 501 c 3 US based charity, to provide free healthcare, education and the administering of positive social change in one of the most impoverished regions of the world. His philanthropic work continues to this day with plans to expand the project into other parts of Africa.
