Certified copies of the files can be ordered by completing a form and mailing it or fax it to the Department of Health. A fee of $5 is charged per copy and $4 for succeeding copies provided they are ordered at the same time. Mail the fee together with the completed form in cash, money order or credit card. Faxed forms are paid in credit card and additional fees are imposed for priority handling and another fee for UPS service. Include your name and address in the form. Fees are not subject no refunds even if no files are available for the subject.
